Leaving school is an exciting time, and our NDIS 1:1 Transition Support program is here to guide students as they take the next step. We provide personalised support to help young individuals build essential life skills, develop independence, and explore their passions.
We start supporting your child during their last year(s) of High School as they transition with us Full Time.
Our tailored services include assistance with daily living tasks, community participation, goal-setting, and personal development.
By focusing on each person’s unique aspirations, we help create a pathway that aligns with their abilities and dreams.
Together, we’ll ensure a smooth transition into adulthood, empowering students to navigate this new chapter with confidence and success.
